New Employee Notification & Service Request
Use this form to notify HTS of a new employee in your department and request services such as email, network logins, and software access IDs.

Access Card Request
Use this form to request new access card (49er Card) rights or to change or stop service on a 49er Card for a Housing employee.

Access Extension Request
Use this form to request the extension of computer network or building access privileges for an employee, contractor, or student worker who will be employed by the Department longer than originally requested.

Employee Termination Notification
Use this form to notify HTS of a departing employee and to request the shut down of that employee's accounts and services.

Please use the Technology Support Request form in Nyumba Staff Gateway to report Technology problems and requests.
